HMAC Membership Requirements

To Apply for HMAC membership,

download the Memebrship form and send to us with required documents. Memberform.doc

OR

Fill Online registration form and send supporting documents via mail

The following must be submitted

  Resume
  Proof of Canadian Citizenship or Landed Immigrant Status.
  Copies of Educational Certificates/Diplomas/Degrees.
  Transcripts Issued Directly by Educational Institution(s)
  Course outline/College prospectus/Calendars, etc.
  2 Letters of Reference from Professionals. (Please include telephone numbers for references).
  $200.00 (CDN) FOR NEW ANNUAL MEMBERSHIP / $75 (CDN) for NEW Student membership.
    $100.00 (CDN) FOR RENEWAL OF ANNUAL MEMBERSHIP .
